Integrated Care Approach to Assisted Living

Residents of The Bristal meeting with a team member

At The Bristal, our approach to assisted living is centered on providing care that is as unique as each resident we serve. We believe there is no one-size-fits-all solution, which is why we develop Individualized Service Plans (ISPs) tailored to meet specific personal needs. This commitment ensures each resident is only paying for the services they truly want and need.

Each ISP is thoughtfully created by our professional care team members with the assistance of residents, their family members, and others in their close circle of care. Designed to be flexible, ISPs adapt as a resident’s needs evolve over time. Our focus is on delivering compassionate, expert support where and when it’s needed, prioritizing comfort and earning trust in a supportive environment.

ISPs, also known as personal care assistance plans, may include a variety of activities of daily living (ADLs) to support a fulfilling, dignitfied, and comfortable lifestyle, such as:

  • Personal hygiene and grooming
  • Dressing and clothing selection
  • Bathing and showering
  • Mealtime needs
  • Mobility assistance
  • Medication management
  • Continence management
  • Transferring support
  • Coordination with physicians and other healthcare professionals

Independent Living The Bristal

The Bristal is able to provide assistance and care to residents within the licensure granted to us by the State Department of Health. In many cases, supplemental services can be brought in to the assisted living residence if they are not provided in-house. If you need more advanced medical care, however, a transition to skilled nursing may become necessary.

Are residents allowed to leave the community for day trips or overnight stays?
The Bristal Assisted Living,

Yes, absolutely. Residents who live at The Bristal are free to come and go as they wish. We do ask that they sign in and out at the entrance so we can be aware of who is in the building at all times, and make the concierge aware of any extended absences – like holidays with family or a vacation.
